What are Community Programs?
Community programs are part of the Home and Community Care (HACC) initiative, designed to support individuals who may face barriers to engaging with their community. Unlike traditional Planned Activity Groups, these programmes are entirely community-based, offering opportunities for participants to take part in activities they might not access independently.
The focus is on building confidence, fostering social connections, and promoting active participation through activities like group BBQs, visits to local attractions, outdoor walks, gym programs, and more. These programs empower individuals to feel included, supported, and engaged in their community.

At arbias, Community Programs are designed specifically for individuals living in Supported Residential Services (SRSs) and are available to those under the age of 65. These programs provide opportunities for people to reconnect with their community, try new activities, and build independence all in a supportive and welcoming environment.
The program follows a person-centred approach in line with the HACC Active Service Model (ASM). This means participants are involved in planning their activities, making choices about what matters to them, and working toward their personal goals. Activities are tailored to be meaningful and engaging, helping individuals feel included and valued.

Our team takes the time to understand the client’s needs and goals. We work closely with SRS managers and staff to gather insights, conduct a risk assessment, and identify the client’s interests. This step ensures we can provide the right opportunities that are both meaningful and safe.

Planning activities
Activities are carefully planned based on the client’s goals and preferences. These plans are shared with the client and their SRS so everyone knows what to expect, creating a sense of clarity and excitement.

With everything in place, the service begins according to the agreed roster. This is where participants can start exploring new opportunities, connecting with others, and feeling part of their community.
