What is Case Management?​

The Transport Accident Commission (TAC) is an organisation that supports individuals injured in transport accidents in Victoria. TAC provides funding for medical treatments, rehabilitation, and other essential services to help people recover and regain independence after an accident. 

TAC Case management is a service designed to provide personalised support and guidance to individuals during their recovery journey. Case managers help clients set and achieve goals, connect with necessary services, and navigate the support systems available through TAC.

With over 30 years of experience, we specialise in supporting individuals with Acquired Brain Injury (ABI) and provide personalised, high-quality support for people with other disabilities, ensuring every client receives the care they need to rebuild their lives after a transport accident. 

Our role begins when we receive a referral from a TAC Support Coordinator or Support Worker. We work closely with clients to conduct an initial assessment, understand their goals, and create a plan tailored to their recovery needs. This plan is then submitted to TAC for funding approval. 

Once approved, we spend the next three months helping clients work towards their goals. This might include connecting them to vital services, assisting with housing, or addressing specific challenges. At the end of the three-month period, we provide TAC with a progress report or exit report to ensure every goal has been addressed and any ongoing support is planned.

How to get support

Step 1

Referral

Referrals can only be made by a TAC Support Worker or Support Coordinator. If you’re a client or family member seeking this service, you will need to speak to your TAC representative to initiate the referral process

Step 2

Initial assessment

Once we receive the referral, our case manager will arrange to meet with the client. During this meeting, we will conduct an assessment to understand their needs, goals, and circumstances.

Step 3

Funding submission

After the assessment, a funding request is submitted to TAC. This request includes the client’s goals and the necessary support hours required for a three-month period.

Step 4

Begin support

Once approved, our case manager will start working with the client to achieve their goals. Progress is reviewed throughout the three-month cycle, ensuring the client stays on track.

Step 5

Ongoing or exit plan

At the end of the three-month period, we provide TAC with a progress or exit report. If additional support is required, a new funding request is submitted to continue assisting the client.
