TAC Outreach Services

Acquired brain injury &
high complex needs specialist services provider

What are Outreach Services?

Outreach Services provide face-to-face support to people recovering from transport accidents. These services focus on helping clients with complex needs, including psychosocial challenges, achieve their goals and rebuild their lives.

Tasks can include assisting with appointments, improving daily living skills, or accessing community resources. Meeting clients in person is a core part of this service, as it helps build trust and provide more effective support.

Outreach Services at arbias

At arbias, TAC Outreach Services provide face-to-face support to clients recovering from transport accidents, focusing on helping them achieve their goals and regain independence. We specialise in supporting people with Acquired Brain Injury (ABI), while also providing personalised care to those with other disabilities.

Outreach Services at Arbias include elements of case management, ensuring clients receive the right support to navigate complex challenges. This involves tasks such as coordinating services, assisting with appointments, and connecting clients to essential resources. Our team meets clients in their homes or communities, offering practical, hands-on assistance designed to meet their specific needs.

Operating on a three-month cycle, we work closely with clients to review progress, adjust plans as needed, and empower them to build confidence and independence. Everything we do is centred on what matters most to the client, ensuring the support provided is meaningful and effective.

step by step guide on

How to get support

Step 1

Referral

Referrals can only be made by a TAC Support Worker or Support Coordinator. If you’re a client or family member seeking this service, you will need to speak to your TAC representative to initiate the referral process.

Step 2

Initial assessment

Once we receive the referral, our case manager will arrange to meet with the client. During this meeting, we will conduct an assessment to understand their needs, goals, and circumstances.

Step 3

Funding submission

After the assessment, a funding request is submitted to TAC. This request includes the client’s goals and the necessary support hours required for a three-month period

Step 4

Begin support

Once approved, our case manager will start working with the client to achieve their goals. Progress is reviewed throughout the three-month cycle, ensuring the client stays on track.

Step 5

Ongoing or exit plan

At the end of the three-month period, we provide TAC with a progress or exit report. If additional support is required, a new funding request is submitted to continue assisting the client.
