We’ll arrange a comprehensive discussion to understand your support requirements and the services you’re looking for.

Step 4

Complete documentation

After discussing your needs, we’ll guide you through completing the required paperwork, such as service agreements, consent forms, risk assessments and consent to coordinate with relevant providers.

Step 5

Start Your support journey

Once everything is in place, a dedicated Specialist Support Coordinator will be assigned to you. They’ll commence services with initial assessment of needs, goals and priorities, followed by coordination planning.
